Any of the explainations could be correct, I would just ask, or say I've never had my picture taken at a doctors office before, whats that for? I would think that they would tell you something.
but if it was a pain clinic, or doctor for pain meds, he or she may wan't to make sure you are not a drug seeker, and come again with a new ID. ( my guess). and as far as being legal, it may differ from state to state, in my state it is ok to record or to take a picture of you as long as the picture taker could have seen or heard you in a area that anyone could have been and overheard you or seen you.
So if the door was closed and your picture was taken, as you had a resonable right to think it was private, that would be illegal, being in a pain clinic, or a doctors office you may have to sign somthing, ( in the fine print) that says you gave permission to be photographed.( how many of us have ever refused signing a document when it come to a doctors visit, you have a level of trust there)
And/or ( if this applies) remember what we signed 2-10 years ago. Most of us just trust the doctor and does what he/she says--thats how we were brought up. IMO
