Oxycontin gives me a rrreally bad headache. I'll second on the Demerol though.

I have a question about the patches: I could not keep them on when they got wet plus I was darn tired the entire time. I know my doc would put me back on them (that is what he wants me to have) but I was just exhausted the entire six weeks.
He told me that there was something he could give me-a stimulate-to offset the sleepy effect but I don't want to take more-I was looking for less.
How do you keep from falling asleep driving (if you do) and work? How do you keep it on -I had to tape it? And what do you do when it is not enough? My PM doc does not "believe" in "breakthrough" pain-

Did I miss something while I was sleeping. I thought methadone could only be used for narcotic addiction, with very strict Rules...but then again I still haven't woken up from the early 70's.

I think #1 IV Dilaudid #2 IV demerol #3methadone. I have not tried the fentanyl patches so they could easily be one of the top 3.

Iam not a sure ,however I read somewhere that fentanyl is many times more potent then all other forms of opiate based pain killers. If anyone has any input or wants to disput that ,please do ,I remember reading something about the potency of fentanyl , and how very strong it was ,however it was a long time ago and things surely do change with time !

The last time I was in the hospital (a few weeks ago), I was on a dilaudid pump, which worked very well, and seemed very strong. I've also had a lot of IV demerol, and though it seems stronger to me, the effects wore off quicker. Also, oddly enough, IV Fentanyl worked really well, but the patch didn't do anything for me AND it made my feet swell up to giant balloon size. Has anyone else ever had their feet swell on Fentanyl? It was weird, and not fun.

The debate about strength is often clouded by the amount of relief achieved by individual patients. This can also depend on delivery and condition. But when it comes to milligram for milligram (or microgram as the case may be) opiates are measured against IV morphine, as it is not synthetic. The PDR comes down this way (administered IV)....
1. Fentanyl. Hands down the most potent opiate to be synthesized. It is measured in micrograms, and is approx 100 times stronger than morphine.
2. Hydromorphone (Dilaudid)
3. Levorphanol (rarely used)
4. Morphine tie Methadone
5. Meperidine (Demerol)
6. Oxycodone
7. Hydrocodone
8. Codeine
9. Propoxyphene (Darvocet)
These are the big ones. On a personal note, I use 300McG/hr of Fentanyl in the form of 3 100McG Duragesic patches and Dilaudid 8mg tablets for breakthrough pain. These are pretty high doses, but I'm a big guy (6'6", 230), and have a pretty messed up back courtesy of a big rig than turned my Tahoe (and my spine) into scrap. I switched to the patches from 320mg of Oxycontin so I could have better long term relief.
Trivia: The absolute strongest opioid produced is called Sublimaze. It is an ultra-potent form of fentanyl that is 1000 times stronger than Morphine, and is used during anesthesia and to sedate large mammals, such as rhinos and elephants.

TVstar - I too have occasional issues with my feet swelling. Also, it took about 48 hours to get the full relief of the Duragesic patches. Also, they don't last the full 72 hours as advertised, so my MD prescribes them q48-72. My insurance does not cover their cost, so I end up paying about $75-100/day for pain relief.

I TAKE 70 MG OF LIQUID METHADONE DAILY FOR CHRONIC BACK PAIN IT WORKS PRETTY GOOD BLOWS HYDRO CAN PERCOCET OUT OF THE WATER ON PAIN RELIEF BECAUSE METHADONE HAS A HALF LIFE 24 HOURS PAIN FREE, BUT CAN LAST UP TO 48 HOURS, TO PEOPLE ADDICTED TO HYDRO AND OXY OR DEPENDENT ON THEM, CONSIDER METHADONE AS AN OPTION, YOUR NOT HIGH AS A KITE ALL THE TIME BUT IT TAKES CARE OF THE PAIN EFFIENTLY AND ALOT BETTER FOR YOUR BODY THAN THY LORTAB AND OXY WITH ALL THAT TYLENOL ALSO IT IS A LOT CHEAPER!!!! METHADONE HAD SAVED MY LIFE, I WAS GOING ER TO ER, I WAS EVEN SUCIIADL AT A POINT, I SPENT 6 MONTHS ON THE STREETS BUT SINCE METHADONE HAVE GOTTEN MY LIFE BACK , IT REALLY HAS DONE WONDERS FOR ME I ASK ANYONE WITH A BAD HYDRO/OXY DEPENDDANCY TO CONSIDER METHADONE

Being in Canada, we have several different formulations that I don't read about here.
For my Chronic back pain I went from Tylenol 3's, to Tylenol 4's until my tolerance built up so that I was getting toxic on the Tylenol. (300 tabs a month)
When I was moved to Codeine Contin - a time relase Codeine-only tablet, at 200mg x 3 per day it solved the problem and I was able to get active again. However, after a year at the maximum dosage, I started to get swelling and fluid retention and my phosphate levels were way to high. My Doc finally moved me to HydroMorph Contin (Purdue) which is a sustained relase Dilaudid supplied in a gelatin capsule full of hard beads of medicine which can actually be opened and poured on your food (swallow, don't chew!). I don't know if this is consistent with the medication that I have seen described here, but I am glad to not have to deal with patches - I hated them when I quit smoking years ago. These capsules come in 3, 6, 12, 18, 24 and 32 mg. I have never been more pain free in years, although I did go through some significant codeine withdrawal since the 12 hours advertised as effective dosage intervals is actually only about 8 hours with me. My dosage has been adjusted several times to 3 times a day, I have 8mg Dilaudids for breakthrough pain, and I am now doing great. (fingers crossed)!
Morphine gives me headaches, and I have never had luck with Oxy-anything, I must not have those receptors. Codeine was more effective than Percocet (Oxy Co-set in Canada) I have never tried Fetanyl or Methadone, but Demerol, Talwin, Buprenorphine and Darvon (propox) are partial antagonists and all send me into some withdrawal and have limited analgesia. All of the most effective that seem to be described here all seem to be pure opiod agonists. Would anyone agree?

Palladone seems to be the big thing now. In it's therapeutic dose, dilaudid would seem to be the winner based on my experience with the board. I'm not impressed with Oxycontin 40's, as I still have to take quite a bit of hydro during the day...even from the get go.

I am pretty sure the Fentanyls are the strongest pain killers in use. There are other opiates even stronger than the Fentanyls (hundreds of times stronger), but they are never used. I think Prodine is one, but I may be getting my opiates confused.
I think levorphanol is only 5 times stronger than morphine, where as Fentanyl is 50 to 100 times stronger than morphine (depending on which Fentanyl).
Maybe someone who is a chemist or pharmacologist knows?
